A Message from the Nurse
If your child is enrolled to attend CCHS, please be advised that a current physical is required for all incoming freshmen. You can use the Diocesan Student Physical form or a NCHSAA physical form will be accepted if your student is participating in sports. If your child needs medication during the school day (prescription or OTC) please also complete the Medication Authorization Form. It is strongly recommended to complete this form, as it is required order for one of the nurses to give your child OTC for mild cramps and headaches and to keep them in school.
Please note: The nursing team does not utilize the athletic platform, Dragonfly, so all completed paperwork should be delivered to CCHS, Attention: NURSE.
