Academic Achievement Program Application
In order to be eligible for Academic Achievement, students must have the appropriate paperwork to support their application. Please note that there are specific types of assessments needed in order to be eligible. The following assessments (if dated within the last 3 years) will be reviewed for eligibility:

SAT consent form for Accommodations Request
Unlike the ACT process, The SAT consent form for Accommodations request needs to be signed so that the SSD Coordinator at Charlotte Catholic can begin the process. This form allows Charlotte Catholic to submit the relevant and confidential documentation to support the request for accommodations.

ACT consent to Release information Form
After the student has created an ACT account, and requested accommodations be placed on your ACT profile, ACT will contact the Academic Achievement Department at Charlotte Catholic.
ACT will ask for relevant documentation to be sent electronically to them. In order to do this, this form MUST be on file in the Academic Achievement Dept.
