Mangus Health Portal - Athletic Paperwork Repository 

If you are planning a trip to your doctor for your 22-23 "sports physical", please bring Pre-participation/Physical exam - Updated April 2021 form for completion by the physician.  Spanish language version of pre-participation/physical exam is available here.

Families new to Charlotte Catholic for the 22-23 school year should expect an invitation from Mangus Health Portal mid-July.  

Dear Parents,

For the upcoming school year, all athletic department medical information is hosted in an online database.  The healthcare management system we have adopted is called Magnus Health SMR (Student Medical Record). As this is a web-based system, you will have continuous access to your child's health record as well as the ability to make updates when needed. You will also have the option to take the digital health file with you after your child graduates.

In mid-July, new to Charlotte Catholic families will receive an email from Magnus Health which provides access to your account.

Timely submission of athletic information and other forms is vital so that preparations can be made prior to the first day of practice. Please reserve 20 to 30 minutes per child, depending on particular circumstances. The good news is that you will not have to repeat this work next year: the data is securely stored, so going forward you will simply provide updates to the record as needed.

To access your Magnus Health SMR Account:

Please visit www.CharlotteCatholic.org and click on the "Athletics" tab. You will see the Magnus link under "M" on the Athletics Directory home page.
