After you and/or your parents have created an ACT account, and have requested accommodations be placed on your profile, ACT will contact the Learning Support Department at Charlotte Catholic.

ACT will then ask LS to submit the relevant documentation electronically to them. In order to do this, this form MUST be on file in the Learning Support Dept.
